ABSTRACT
Aim:
The aim of this case report is to present the treatment of two high angle Class II division 1 cases demonstrating severe gummy smile via orthognathic surgery.
Case Report:
The first case is a female with skeletal and dental class II malocclusion, increased mandibular plane angle and severe gummy smile. After the pre-surgical decompensation orthodontic treatment phase an orthognathic surgery comprising maxillary impaction and mandibular advancement was applied. The second case is a male presenting skeletal and dental Class II malocclusion and increased anterior facial height. He has a convex soft tissue profile, gummy smile and incompetent lip closure. It was decided to extract four second premolars. Subsequent to extractions, presurgical orthodontic treatment was applied and maxillary impaction, mandibular advancement and genioplasty surgery was performed.
Conclusion:
The occlusal and intermaxillary relationships were corrected after the double jaw surgery in both of the cases. The lower anterior facial height was reduced significantly and a more balanced soft tissue profile was achieved.
